The Maine Hospital Association (MHA) has created a standard form that most people in Maine use to create their health care advance directives. The MHA also provides instructions for helping you to understand and complete your advance directives.
We, at Legal Services for the Elderly, have created an interactive version of the form that you can fill out online and then print.

LSE's interactive version of the Advance Directives form (fill out online)
This is a large .pdf file that may be slow to download. To use the form scroll to the first blank line, type in your answer, then tab to the next blank line. Or use your mouse to move around the form. Click on your mouse to fill in check boxes.
If you are having trouble viewing the form or you're getting an error message, try another browser or click here to visit PTLAs webpage for more info on using .pdf fillable forms and get suggestions on how to get the form to work with your computer or in your browser.
Blank Advance Directives form (print and fill out by hand)
Use this form if your computer software cannot handle LSE's fillable form.
